Referral Form found below and send it to administration@tepaepae.co.nz or  alternately you can collect a Referral Form directly from our office at

330 High Street, Level 5, Lower Hutt, 5010.

Walk in referral times CLOSE at 4:00 pm daily Monday - Friday.
